Eligibility

  • All full-time staff who work at least 30 or more hours per week

  • Benefits for new hires will become effective on the first of the month following 30 days of full-time employment

  • Dependents include legal spouse or domestic partner, child(ren) up to age 26 and disabled child(ren) of any age

  • If you do not enroll during your new hire enrollment period, you will have to wait until the Open Enrollment period to enroll unless you have a Qualifying Life Event

  • Benefits for new hires will become effective on the first of the month following 30 days of full-time employment

Qualifying Life Events

Outside of New Hire and Open Enrollment, your only other opportunity to make changes to your benefits is if you experience a Qualifying Life Event (QLE). These events are significant life events like getting married, having a child or a dependent losing other coverage. If you experience a QLE, you MUST submit your request for change and documentation to Human Resources within 30 days of the effective date of the event.

Medical Plan Options

You have two Nippon Life medical plans to choose from: a Base PPO plan and a Buy-Up PPO plan. Both plans offer in- and out-of-network coverage, but deductibles and out-of-pocket maximums differ. Review the Summary of Benefits and Coverage (SBC) for each plan below.

Dental Options

Dental options are administered through Aetna. For dental, you have the choice between a Dental HMO plan and an Dental PPO plan. Both offer comprehensive dental coverage with the DMO plan offering fixed copays with in-network providers only.

Dental and Vision Options

Dental and vision options are administered through Guardian. For dental, you have the choice between a Base plan and an Upgrade plan. Both offer comprehensive dental coverage with the Upgrade plan offering richer benefits. For vision, the comprehensive plan offers yearly coverage for lenses and frames or contact lenses.

Savings Accounts

SHL Medical offers a Health Savings Account (HSA) that pairs with our HDHPs, including an employer contribution. We also offer four Flexible Spending Accounts (FSA): Healthcare FSA, Limited FSA, Dependent Care FSA, and Adoption Assistance FSA. Each account has different eligibility requirement and contribution limits set by the IRS.

Life and Disability Benefits

SHL Medical offers Company-Paid Basic Life and AD&D Insurance to eligible employees in the amount of $50,000. During open enrollment, you may elect to purchase optional life insurance in varying amounts for yourself and your dependents.

SHL Medical offers Company-Paid short-term and long-term disability coverage. This coverage helps replace a portion of your income should you be unable to work due to a non-work related illness or injury.

Voluntary Benefits

During open enrollment, you may enroll in additional voluntary benefits like Critical Illness, Accident, Hospital Indemnity, or Cancer Insurance.
