Medical Plan Options
At DSA, we hope to inspire employees to take charge of their health. That’s why we offer two great medical plans from Nippon Life to choose from: a Base PPO plan and a Buy-Up PPO plan. Both plans offer in- and out-of-network coverage, but deductibles and out-of-pocket maximums differ. Review the Summary of Benefits and Coverage (SBC) for each plan below.


Dental Plan Options
Our dental plans are administered through Aetna. You have the choice between a Dental HMO plan and an Dental PPO plan. Both offer comprehensive dental coverage with the DMO plan offering fixed copays with in-network providers only.

Vision Plan Options
Our vision plan administered through Aetna, offers comprehensive yearly coverage for lenses and frames or contact lenses. The group number for this plan is 141236.

Flexible Spending Accounts
DSA offers two types Flexible Spending Accounts (FSA) through Chard Snyder: Healthcare FSA and Dependent Care FSA. FSA benefits run on a calendar year basis and you need to use your funds before December 31st each year. You can roll over a certain amount set by the IRS and use it to pay for eligible expenses in the next plan year. Any amounts in your Healthcare FSA that exceed the IRS rollover limit will be forfeited.
Healthcare FSA: This account lets you set aside pre-tax funds to pay for qualified healthcare expenses like copays and prescriptions. Contribution limits are set by the IRS each year.
Dependent Care FSA: This account lets you set aside pre-tax funds to pay for qualified child care and elder care expenses. Contribution limits are set by the IRS each year.

Life & Disability Benefits
DSA offers Company-Paid Basic Life and AD&D Insurance from Mutual of Omaha to eligible employees in the amount 1x your annual salary up to $150,000. During open enrollment, you may elect to purchase voluntary life insurance in varying amounts for yourself and your dependents.
DSA offers Company-Paid long-term disability coverage from Mutual of Omaha. This coverage helps replace a portion of your income should you be unable to work due to a non-work related illness or injury.

Voluntary Benefits
To ensure your family is financially protected, you may enroll in additional voluntary benefits from Colonial Life like Accident, Critical Illness, or Short-Term Disability Insurance.
Enrolling In Voluntary Benefits: New hires may enroll in voluntary benefits within 30 days from when they become eligible for benefits. To streamline this process, Colonial Life representatives are available to guide you through enrollment.
